Appointment requests
Submitting a form is a request, not a confirmed appointment. We confirm every appointment by phone or email. Please do not use forms for urgent needs; for a knocked-out tooth or facial swelling, call us or go to the emergency room.
Cancellations
Please give us at least 24 hours’ notice to cancel or reschedule. Late cancellations and missed appointments may be charged a $25 fee, which we waive for illness and emergencies.
Prices and payment
Prices listed on this website are in U.S. dollars and may change. The price that applies is the one in effect on the date of your visit. Coverage depends on your dental plan; we will check benefits when asked but cannot guarantee what your insurer will pay.

Limitation of liability
